Instructions:

• Identify youth in your church. You can list individually or as a group. For example, youth in my community or youth in the choir
• Objectively weigh how the individual or group views you based on each Element. Be honest!
• Enter values 1, 3, or 5 only
• 1 is low on the rating scale, meaning there is opportunity for immediate improvmement
• 3 is moderate on the rating scale, there is some room for improvement
• 5 is high on the rating scale, meaning there is little to no room for improvement. Just keep doing what you're doing
• The assessment will give you a score and identify the areas where you are proficient and where you need to invest time
